Patient Information Needed

  • Patient history
  • Age, height, weight
  • Whether you have had injections?
  • Whether you are taking anti-inflammatory medication?
  • Whether you are diabetic?
  • Do you have any heart problems?
  • If you have had prior surgery?

X-Ray must be:

  • Taken within the past 6 months.
  • Weight Bearing Films

Mail films or CD to:

Once the films are received the doctor will review them and make recommendations.
